A recently released paper looking at systematic approaches to program adaptation of evidence-based health promotion programs focused on the computer-based sexuality education program, The World Starts With Me (WSWM), for a case study.

WSWM, introduced in 2003 by the World Population Foundation, was developed for a priority population in Uganda and adapted for use in Indonesia this year. The program’s target population includes both school-going young people and early school leavers, generally ages 12-19, and is complemented with teacher-led activities. It utilizes a comprehensive approach that includes building IT-skills and creative expression, aiming to contribute to sexual and reproductive health as well as social and economic development.

The program provides introductory computer skills lessons as well as 14 lessons on adolescent development, decision making and sexual and reproductive health and rights. The lessons employ effective sex education methods, including practical applications to increase knowledge, develop attitudes, and help youth recognize and cope with social influences.

The paper’s authors, all public health professionals and academics, chose WSWM as their case study because of its proven success. In 2004, the program received the Golden Nica Award by Prix Ars Electronica in the digital communities category, and UNESCO used WSWM’s digital curriculum as a guideline for implementing effective sexuality education.

A meta-analysis revealed that computer technology-based programs like WSWM have similar results to traditional human-delivered interventions in terms of HIV/AIDS prevention behavior adoption. In particular, they proved to have positive results in increased condom use and reduced sexual activity, numbers of sex partners and sexually transmitted infections (STIs). Computer-based assignments also allow programs like WSWM to be student-driven, which means teachers are less burdened to talk about sensitive sexuality issues which may be uncomfortable for students. According to the meta-analysis, given computer-based programs’ “low cost to deliver, ability to customize intervention content, and flexible dissemination channels, they hold much promise for the future of HIV prevention.”

This research paper reveals that ICT-based HIV-prevention programs like WSWM can be effectively adapted in other contexts, which could lead to widespread reduction in HIV rates among young people.

 

 

 

CrowdOutAIDS, the online crowdsourcing project that engages young people in developing a UNAIDS strategy on youth and HIV, has wrapped up its fifth week. The project launched in October 2011 and will run for two months, with the final crowdsourced strategy to be produced in January.

Crowdsourcing is a technique used to quickly engage large numbers of people to generate ideas and solve complex problems. CrowdOutAIDS’ target “crowd” is young people, 3,000 of whom become infected with HIV every day and 5 million of whom currently live with the virus.

The project’s approach is to follow a four-step model:

  1. Connect young people online
  2. Share knowledge and prioritize issues
  3. Find solutions
  4. Develop collective actions on HIV

Once the fourth step is completed, the UNAIDS Secretariat will put the youth strategy into action, and the strategy could become an advocacy platform in future UNAIDS work.

Currently the project is in the second stage of sharing knowledge. Youth from all over the world have been connected through eight regional Open Forums that are in Arabic, English, Spanish, French, Russian and Chinese. The moderator of each forum starts each day with a question (such as “What is your description of a healthy relationship?”) and participants respond and interact with one another.

CrowdOutAIDS steps

The first week of the project revealed some of the major problems, in the eyes of youth participants, with UN agencies’ current approaches to working with youth. Participants expressed concerns that UN initiatives of working with youth in HIV response lack strategic vision and have no clear plan, and hinder young people from participating in decision-making.

Last week’s mHealth working group meeting laid out the opportunities, challenges, and potential of monitoring and evaluation (M&E) within the mHealth sector.

Several experts in the field presented their experiences of monitoring and evaluating mHealth projects, emphasizing the considerable potential that mobile projects offer in generating robust and accurate data. Kelly L’Engle, a behavior scientist at FHI 360, discussed the need for M&E in order to gauge the impact of mHealth. She claimed that mHealth technology is not being fully leveraged and that the current mHealth research “doesn’t provide evidence on actual impacts…or answers to critical research questions…”

James BonTempo from Jhpiego presented on evaluating behavior change evidence from a text message project that promotes male circumcision (MC) for HIV prevention in Tanzania. He referred to this evidence extraction as “mining the data exhaust” – that is, the data generated as a byproduct of ICTs, the “trail of clicks” that ICT users leave behind.

The MC program offered a toll-free text-messaging/SMS service. With the service, individuals could request to receive information on the benefits of MC (using key word: TOHARA), where to find MC services (WAPI), as well as receive follow-up care information after undergoing the circumcision (BAADA). While the SMS platform was intended to capture requests to the three keywords and generate access logs for system performance monitoring, Jhpiego has analyzed this data in order to see if there was a relationship between requests for the MC data and actual utilization of MC services.

The data set included 12,056 keyword requests sent by 4,954 users. After performing a Chi Square analysis on the data, the project team found that requests for information on the benefits of MC (TOHARA) was not strongly associated with going to receive the circumcision. However, it was found that those who requested where to find the data (WAPI) did have a statistically significant association with undergoing the circumcision. These results are consistent with pathway models of behavior change, indicating that those who simply wanted to know more about MC were not quite prepared to undergo the procedure, but those looking for service availability were ready to use the services.

The associations found in this particular project imply that providing text or voice messages that tell people where to get a particular service could be more effective in encouraging clients to utilize that service. M&E that finds results such as these could help project leaders design SMS services that generate more useful data.

Like most forms of M&E, there are limitations on findings that use mobile data. There is a lot of information that mobile devices do not provide, such as which phones belong to whom and who actually sends the text messages; this makes it difficult to link messages to specific individuals. In the MC case, data analysts found an association, but did not necessarily know the nature of the association. It can also be challenging to find the time or manpower to rake through the massive amount of data that is produced by mobile devices.

While M&E in mHealth has its limitations, it is difficult to find data that can be collected and analyzed as quickly, cheaply, or easily using other means. Paper, radio, and television simply do not offer the same kind of easily-mined data exhaust that mobile does.

 

 

Digital Health 4 Digital Development was the theme of choice for the 2011 South-South awards that took place earlier this week. The United Nations-supported awards ceremony, held September 19th, honors governments, organizations and individuals accelerating progress toward the Millennium Development Goals (MDGs), and this year awards were given for utilizing ICTs for the advancement of the MDG health goals.

The awards were organized by South South News, a digital media platform launched by the United Nations General Assembly High-level Committee in 2010. The platform disseminates development news and allows countries in the global South to share best practices in advancing implementation of the MDGs. As health remains a high priority within the international development field, with three of the eight MDGs calling for health improvements by 2015, this year South South chose the “catalytic and unifying force of ‘digital health’” as its focus.

Awardees were recognized for policies, programs and projects that address global health using ICTs as a mechanism for “scalability and replicability of the broad development agenda.” Awards were given on health categories such as Women and Children’s Health and HIV/AIDS, TB and Malaria with special consideration given to achievements in the fields of mobile, broadband, Internet, medical, and pharmaceutical applications.

Among the winners was Bangladesh’s prime minister Sheikh Hasina who received recognition for the use of ICTs in addressing women and children’s healthcare. The government of Bangladesh has made increasing the use and availability of ICTs a priority over the past decade, declaring IT a “thrust” sector and implementing a national ICT policy in 2002. The prime minister has been a catalyst for ICT development in Bangladesh, launching the Digital Bangladesh 2021 program, reducing taxes to make computers and other electronics more affordable, and giving free internet services to schools. This is not the first time she has been recognized for her ICT work in the country. The PM noted during the ceremony that most of Bangladesh’s 11,000 community health clinics have been digitized, allowing for free treatment of diseases like malaria and AIDS and reducing maternal and infant mortality rates.

Other development agencies and UN-affiliated organizations are also choosing to give awards to those using ICTs to advance public health initiatives. The Elena Pinchuk ANTIAIDS Foundation, rising from the work of UNAIDS High Level Commission on HIV Prevention, has launched a competition to find innovative start up projects that use social media and mobile phones for HIV prevention. The competition is taking applications until October 1st and the winners will receive up to $10,000 to implement one-year projects.

As global health becomes a pressing priority, it is essential to utilize, innovate, and increase access to ICTs within the healthcare sector. The South South awards have set a precedent by recognizing the success of ICT4Health in accelerating the Millennium Development Goals and could encourage Global South countries to follow in the footsteps of nations like Bangladesh.

 

Argusoft, a Fremont, CA start-up that’s combines video, instant messaging and Internet telephony in a platform for “e-health” programs in the developing world, is ready to implement a mobile phone application that will enable field workers to register HIV-positive mothers and provide regular updates on their care.

The application, called mAID, runs on any java-enabled phone and utilizes the SMS interface. It is designed primarily for health workers that go out into rural communities to inform citizens on different health issues.

Using the application in the field is simple. Cell phones are given to health workers who communicate through the phone to a central database. The health workers are sent daily instructions in the morning via SMS on where to go and which houses to visit. The health workers collect relevant data on HIV/AIDS prevalence and awareness and report the data back to the central database via SMS.

The new program is overseen by the Indian government with financing from the Global Fund, a nonprofit in Geneva that targets AIDS in developing countries. IL&FS, an infrastructure development conglomerate based in Mumbai, is handling logistics.

mAID underwent a pilot test where 35 health workers reached over 2500 patients using the application. The pilot phase ended last month, and based off its results, the Indian government wants to inject 3600 more health workers into the field with the mobile app for a nationwide scale up. There are even talks about using the application in Africa.

The CEO of Argusoft, Ram Gopalan has cited the difficulties of preventing HIV/AIDS as the impetus of his application. “It’s fully preventable, but one of the highest killers of children in the Third World,” Gopalan said.

Gopalan echoes the same sentiments of the Indian Government who have been working to prevent prenatal HIV transmission since 2002, using counseling and testing centers around the country. The Indian Government also cited issues with health worker capacity. Regarding that Gopalan said, “There was a lot of inefficiency, workers misinterpreting instructions, and paperwork getting lost.”

Argusoft is no stranger to implementing eHealth initiatives. In the eastern Indian state of Tripura, Gopalan linked isolated villages with a hospital in the state capital where doctors can remotely diagnose simple but life-altering problems such as cataracts. This telemedicine project has provided eye care for more than 100,000 patients over the past five years.

In the future, Gopalan wishes to introduce a network of accredited family-care doctors from India who could be available 24/7 for live video chats with patients. This is similar to the meradoctor project which already exists in India. In the meantime, the HIV/AIDS burden in India needs some attention as it is the third highest burden in the world in terms of sheer numbers living with HIV.

As South Sudan prepares for independence, the celebrations will be tempered by the grim reality that awaits them. The nation is plagued by numerous health burdens due to decades of civil war resulting in a lack of trained health workers and poor infrastructure, and inadequate health and education systems. One such burden is HIV/AIDS. Dr. Wichgoah Piny, the state’s HIV/AIDS commissioner said that about 116,000 people are known to be infected by the virus in South Sudan, 46,000 of which are being treated at hospitals within the region. This number lies in the middle ground when looking at the rest of Africa. East Africa generally has a higher prevalence whereas West Africa exhibits a lower prevalence of the disease. Some estimates say up to 4.7 million in South Sudan are at risk of acquiring the disease.

But those numbers could grow exponentially in the coming years. The World Health Organization (WHO) has described HIV/AIDS prevalence in South Sudan as “a ticking time bomb” and a threat which needs a cooperative and collective effort to fight in the new nation. Dr. Olivia Lomoro, the GoSS Undersecretary in the Ministry of Health who spoke at a South Sudan AIDS conference warned, “HIV/AIDS is a real issue in Southern Sudan despite the efforts we have put to fight it. It remains a threat and a time bomb we are expecting.”

Dr. Mohamed Abdi, the WHO Director in South Sudan said that, “AIDS is a big problem in South Sudan and we need to fight it together.” He said that in more than two years he had worked in the region, very few people were getting treatment.

The consequences of a surge in HIV/AIDS could be catastrophic. The health infrastructure is already poor as it stands. An HIV/AIDS epidemic would paralyze the health sector for years and impair growth in other sectors. This could result in a crisis that no new government would be prepared to handle.

So in a sense, this is a defining moment for the future of health in South Sudan. As the new government establishes itself and its policies, it has the opportunity to be forward looking in the development of its health infrastructure. If HIV/AIDS is afforded a chance to “blow up” as WHO fears, the country will immediately be sunk further into a health crisis.

This presents an opportunistic entry point for the implementation of ICTs into the health sector in South Sudan.

Opportunities

A survey completed in 2006 by WHO revealed that over 910 health facilities existed in South Sudan. At the time, a majority of those facilities were claimed to be in a “deplorable state”.  However, Dr. Lomoro recently pledged that Lakes state now has a new hospital which is going to be used as a teaching hospital to help in delivering quality health. She also mentioned investments through the Chinese government will be used to build new health centers and rehabilitate old ones. Incorporating ICTs in the rehabilitation process will pay dividends for the health sector in the future.

South Sudan is being held captive by a number of health issues such as measles, yellow fever and tuberculosis to name a few. Furthermore, South Sudan has up to 80% illiteracy rates in some parts, which make communicating through ICTs invaluable. The most basic forms of ICTs like radio, television and mobile phones can build a communication bridge for the illiterate to keep them connected and informed.

Dr. Lomoro also pointed out that the first target for the Ministry of Health in the Government of South Sudan is the training of health personnel in all the ten states of South Sudan in order to deliver quality health services to the population in a professional standard. One way to go about this would be to include ICT training in that workforce development. Taking this route would build workforce capacity and bolster the development of ICTs in the country.

The population returning from Sudan proper and surrounding African countries are also coming back with health work force skills. So there is a hope that as people return, their skills will be used in improving service delivery.

Several factors exist that support the idea of integrating ICT into the health infrastructure of South Sudan

Present ICT Usage

The market for ICTs is ripe and growing in South Sudan. The initiatives that would make a big impact in fighting HIV/AIDS using ICTs utilize mobile phones and radios as the media for communication. These media platforms are the largest in South Sudan.

A 2007 survey entitled “Media Access and Use in Southern Sudan,” showed that radio was the main source of information for the population as a whole with 59% of respondents citing the radio as a source of information, the highest out of all forms of media. Additionally, HIV/AIDS awareness radio programs make up 17% of the most popular radio programs. This indicates that HIV/AIDS discussions already have a base in South Sudan media.

Mobile phones on the other hand don’t enjoy such high rates of usage as radio due to 30% coverage rates. However, the mobile phone network has expanded considerably since 2005 and is predicted to keep expanding in the near future. Also noteworthy is that only 14% of South Sudanese get their information from newspapers and 13% from television according to a report. It’s clear that radio and mobile phones are the most widely used avenues of communication in South Sudan.

Plans to expand broadband cables

As recently reported, South Sudan is on the shortlist to receive fiber optic broadband cables. Broadband services make it possible to interconnect affiliated healthcare facilities around the country so that they can utilize and share scarce human and technical equipment resources to deliver quality and affordable healthcare services. These cables can establish a foundation for ICTs not just in health, but for all sectors in South Sudan.

Past Successful HIV/AIDS Programs

South Sudan won’t be the guinea pig for using ICTs for HIV/AIDS either. There have been quite a few successful HIV/AIDS programs in the past that succeeded in similar environments:

  • Targeting Nomadic Populations – USAID and other external NGO’s worked together in Nepal in 2005 to introduce a radio program on HIV/AIDS awareness by integrating entertainment into its programming
  • Capitalizing on existing media penetration:

o        Radio – Tanzania also introduced the Tunajali HIV/AIDS Care and Treatment Radio Program in 2010 with the help of PEPFAR.

o        Mobile – Two mobile phone programs, Text to Change(TTC) and Freedom HIV/AIDS used mobile phones to reach thousands of people to educate them on HIV/AIDS

South Sudan should approach some of the major NGO’s on the ground to help implement these initiatives that use ICTs. Organizations like USAID, WHO, UNICEF and the UN have spearheaded many initiatives like the ones described above in the developing world. They should at the very least try to do the same in South Sudan. They need to realize the opportunity at hand.

Possible ICT Policy

So the framework to set South Sudan off on the right foot is present just as they kick off their sovereignty. However, an HIV/AIDS epidemic is a threat. The markets for intervention exist, proven initiatives exist and they can be adopted using the aforementioned plans to establish new broadband cables. All of the stars have aligned. The tricky part is getting officials to the table and churning out an action plan. This will involve NGO officials talking with South Sudanese Government officials and making this opportunity a priority.

South Sudan has already seen its darkest days. They want to move now from being a hub for relief efforts to a hub for development. The health sector can benefit tremendously from using ICTs to prevent an HIV/AIDS disaster. Once HIV/AIDS is under control, the ICTs can be utilized strategically to tackle other health issues, as they are being done all over Africa and the developing world. If not, it may not be long before South Sudan encounters its darkest days once again.

Text to Change (TTC), an mHealth non-profit organization based in the Netherlands, announced earlier this month that they will receive a €2.7 million grant to expand its services. TTC provides an SMS-based educational service to improve the health of citizens in eight countries in Africa and one in South America.

Already a big contributor in mHealth development, TTC hopes to become a leader in the field with the reception of its multi-million Euro grant from the Dutch Ministry of Foreign Affairs via Connect4Change (C4C), a consortium funded by the Dutch Ministry of Foreign Affairs that develops mobile based solutions on issues of poverty in Africa and Latin America. TTC will partner with C4C to expand its services to 11 more countries in Africa and South America by the end of this year.

Implementing ICT in the 11 countries is a top priority for both TTC and C4C. They are hoping the mutual partnership will make establishing ICT services an easier task as the expansion continues. According to TTC, the game plan calls for TTC to provide “low” technologies like SMS and mobile voice services while C4C provides “high” technologies such as mobile internet and video transfer. Therefore, TTC and C4C will play different roles.

C4C will also invest its time reaching out to local entities on the ground to strengthen ICT networks. TTC will focus on improving health outcomes through their established mobile phone initiatives.

TTC sets up their mobile platform through the recipient country’s mobile service infrastructure already in place. They then subscribe mobile phone users to their programs which use SMS communication to inform people of HIV testing, treatment clinics, and other health related services at no cost to the recipients.

TTC programs offer the information through a free educational quizzing service where participants are quizzed about a specific health topic. As participants answer the questions correctly, they are sent more rounds of questions, again at no cost. If the participant can answer enough questions correctly, he/she receives incentives such as phone credit, t-shirts and health products.

Thus far, TTC has reached thousands of individuals with their programs on HIV/AIDS, malaria, and reproductive health. Furthermore, to assess the impact of ICT in the countries they are currently working in, TTC will even conduct large scale ICT evaluations over the next few years in those countries.

TTC is poised to make an impact in ICT through their mobile services. Their work is just another example of how mobile phones are being used as a medium to educate, inform and save lives. The tag-team partnership with C4C will be expanded to all 11 target countries by the end of this year. However, we will have to wait some time before confirming the outcome of this joint strategic approach.

The PATH initiative, which advances malaria vaccines, recently predicted that over ten times more vaccine storage will be needed in the developing world over the next 15 years. However, transporting and managing vaccines in developing countries faces many obstacles, including inhospitable climates, poor infrastructure, lack of electricity and disparate communities.

As a result, over 1 million people die every year in developing nations due to vaccine-preventable diseases. In some cases, vaccines may be reaching these populations, but when they do, they are no longer functional because they were not adequately cooled. Most vaccines must be stored within a certain temperature range in order to remain functional. Health organizations are increasingly turning to technology to help them do just that.

Many NGOs and ministries of health make use of mobile vaccine refrigerators that harness solar energy to power the internal chamber that holds the vaccines. A particularly innovative and resourceful vaccine refrigerator came on the backs of camels. Naps’ Camel Fridge was designed back in 2005 and operates off of solar power. The fridge uses solar panels to harness energy, and sits on the backs of camels that transport the fridges to rural destinations. Over 1,500 of these camel fridges have been sold to WHO and UNICEF programs.

But if a shipment of vaccines arrives spoiled, it is important to know where in the logistics chain the temperature was breached so that changes can be made to prevent a recurrence.

However, some issues with these mobile refrigerators are temperature maintenance and vaccine spoilage. A practical gadget that has made cold chain storage more efficient is a technology that involves vaccine monitoring. SmartConnect, a technology developed by Inveneo that uses existing cell phone networks for data communication, can monitor vaccines by sending information about the arrival time and temperatures of vaccines in transit. That way, officials will know not only if a vaccine has spoiled, but when and where the problem may have occurred.

SmartConnect is not the only project making use of SMS technology. True Energy’s Sure Chill Vaccine Refrigerators, which are WHO pre-certified, can store vaccines at a constant optimal temperature, without power, for up to ten days while using a vaccine vial monitoring system that records temperature changes and can send an SMS to indicate those changes.

True Energy has already shipped multiple units to Senegal and Vietnam under project Optimize, a WHO/PATH collaboration, and aims to ship many more units to countries like South Sudan, Kenya, Nigeria, Yemen, India and many more.

Incorporating mobile monitoring technologies and improving the efficiency of the fridges could make a tremendous impact on the success of many vaccination programs.  And if PATH’s prediction rings true, they will be greatly needed.
